Clinical Analysis of the Gervical Fracture and Dislocation


Abstract
51 patients with cervical fracture and dislocation who had been treated at Chonbuk National University Hospital from March 1980 to August 1984 were clinically analysed. 25 patients of them were managed with surgery by anterior approach and the others(26 cases) were mamged with conventional therapy by Halter or Crutchfield tong traction. The results were summarized as follows.
1 . Of all 51 patients, male were 44 and female were 7 cases. The male to female ratio was 6 : 1. About 50% of cases(26 cases) were between 5th and 6th decades.
2 . The most common site of cervical fracture and disloca.tion.were the level of C5-C6.
3 . Regarding the causes of cervical fracture and dislocation, 33 cases were fall down(65%), car accident in 13 cases(25 % ), others in 5 cases(10 % ).
4 . the rupture of pcsterior longitudinal ligament and protrusion or rupture of disc material
into cervical spinal canal were observed at the time of operation.
5 . The clinical symptoms were disproportionated to the degree of dislocation.
6 . The complete loss of motor and sensory function at injury prevented the improvement of
the sign and symptom inspite of operative and conservative treatment. .
7 . The anterior interbody fusion --.-as effective in the cases with locked facet, severe
destruction and dislocation of vertebral body which dangerous to redislocation.
